At Urban Medical Recruitment we understand that sometimes it can be time consuming for our Locums to collect hard copies of documents which have been sent to them.

To simplify this process, all of our documents can be downloaded in printable versions on our Website.

Locum Timesheets

Each week, we require our working Locums to complete a time sheet for their hours worked and have this timesheet signed by their Manager or Senior Member of staff who they report to.

Each week, our Compliance and Administrative Team will send you out two timesheets or alternatively, you can download a timesheet to complete.

Registration Form

As part of our compliance requirements, all Locums must complete our Registration Form. Our Registration Form is built up of several questions asking you to provide personal information, information with regards to your qualifications gained, work experience, reference details and other general information covering work requirements and location.

All sections must be completed and sections where signatures are required, must be completed.

The new starter form, that each candidate must complete prior to starting work with Urban Recruitment.
